Youth Homelessness Demonstration Program - YHDP
Since 2022, SchoolSeed Foundation, in collaboration with the Community Alliance for the Homeless, has significantly impacted the lives of dozens of families experiencing homelessness through the Youth Navigation Program. This initiative specifically targets individuals aged 18-24, providing crucial support and resources to prevent homelessness and assist those who are already homeless. The program offers comprehensive services, including access to emergency shelters, transitional housing, and permanent supportive housing. Additionally, it provides educational and employment opportunities, life skills training, and mental health services, ensuring that young people have the tools and support they need to achieve stability and self-sufficiency.
